How to Book an appointment

Please complete the form below. I will review it and will respond to you via email. Depending on my schedule, I will get back to you within a weeks timeframe. I do require an appointment fee of $20. We will go over that after I have reviewed your project request.

Once the fee is received, you will get a confirmation of the time and date of your appointment.

Appointment fees are non refundable, you may request a reschedule up to 48 hours before your appointment date.

I do not tattoo minors. Please bring a valid form of Identification to prove that you are 18 or older.
